FORM-100:  Application for Registration of Broodmare

1: All in-foal mares that the foal is intended to be Certified as a Nebraska-Bred horse must be registered with this form before she foals.  To avoid a late fee, this form needs to be at the NTBA office no later than September, 1st.  
2:  If you buy this mare privately after the September 1st date you will be assessed a late fee.  Exception to this is if the mare is purchased in-foal and is not currently registered, then you have 60 days to register the mare to avoid the late fee. The mare has to be in the state for 90 days before foaling and must be registered before she foals. 
3: If you purchased the mare from a nationally recognized sale you have 60 days to register mare to avoid penalty. The mare must reside in Nebraska for 30 days prior to foaling and must be registered before she foals.  
NOTICE: A PHOTO COPY OF THE FRONT AND BACK OF THE CERTIFICATE OF FOAL REGISTRATION (Jockey Club Papers) MUST ACCOMPANY THIS APPLICATION.
When all the requirements are met with the Registrar (NTBA) we will issue this certificate for your records that the mare has officially been registered as eligible to produce a Nebraska Bred Foal.

FORM-125:  Application of Transfer of Broodmare

1: This form is used if you purchase a mare that has already been registered with Form-100. 
2: If purchased after the September 1st date there will be a late fee assessed, with the exception being  in-foal when purchased. If in-foal you have 60 days to transfer her at the regular fee.  The mare must be continuously in the State of Nebraska for 90 days prior to foaling.  The mare must also be registered before foaling.

FORM-130: Broodmare Leaving Nebraska

1: This form is used if you have a registered broodmare that needs to leave Nebraska. 
2: A registered mare can only leave the state for 2 reasons.  1: She is entered into a National Recognized Sale, 2: For Medical Reasons. 
NOTICE: This form must be filed within 3 days of the mare leaving the state.

FORM-135: Broodmare Returning To Nebraska

1: This form is used if you have a registered broodmare that is returning to Nebraska from using FORM-130. Verifying documentation for leaving must accompany this FORM. (Sale Page, Vet Report) 
2: This form must be filed within 3 days of the mare returning to the State of Nebraska. 
NOTICE: This form must be filed within 3 days of the mare leaving the state.

Stallions

FORM-200: Registration of Stallion

1: This form must be registered by the stallion owner, who must reside and stand in Nebraska before stallion is put into service.  If the above criteria have been met then that foal is eligible to be a Class A Nebraska Bred. 
2: If the above criteria are not met then the foal is only eligible to be a Class B Nebraska Bred. 
NOTICE: IF STALLION IS LEASED, ATTACH CERTIFIED OR PHOTOSTATIC COPY OF LEASE AGREEMENT  A PHOTO COPY OF THE JOCKEY CLUB PAPERS AND FEE MUST ACCOMPANY THIS REGISTRATION 
When all the requirements are met with the Registrar (NTBA) will issue a certificate for your records that the stallion has officially been registered as eligible to produce a Class A Nebraska Bred Foal.

Bred Application

FORM-300: Certified Nebraska Bred Application

1: This application will be use for the foal of the Nebraska registered mare upon all requirements being met from Statue 2-1213. 
A certificate will be issued to the foal of the Nebraska registered mare upon all requirements being met from Statue 2-1213. This certificate will make the foal a Certified Nebraska Bred horse.

Required Reports

Required Reports are Required

If required reports are not on file, the Nebraska Bred Certificate of Eligibility for a foal cannot be issued. Late filing of required reports may delay processing of the Breeder's Certificate and the issuance of a Nebraska Bred Certificate of Eligibility, and require a payment of a late fee.

Annual Broodmare Reports:

An Annual Broodmare Report including the ownership of the mare, the location of the mares, the name of the stallion to which the mare was bred and the date of first and last service is due September 1.

Annual Stallion Reports:

An Annual stallion report of mares bred by Nebraska registered stallions is due September 1.

Out of State Stallion Reports:

Outstate Stallion Reports: A stallion report for each broodmare bred to a stallion standing outside Nebraska, or bred to a stallion standing in Nebraska that is not registered with with the official registrar is due September 1.
NOTICE: Late fees will be accessed if the required Annual Broodmare Report is not provided to the official registrar by September 1

Rules

REQUIRED
All broodmares and stallions standing for service in NTBA Nebraska shall be registered with the Official Registry to qualify their resulting foals for certification as "Nebraska Breds".
Nebraska Bred foals are eligible to 
  1.  Participate in races restricted to Nebraska Bred horses.
  2.  Earn purse supplements for their owner.
  3.  Earn breeders awards for the registered breeder and stallion awards for the owner of the stallion at the time of conception

State Statue Rule

(2) For purposes of this section, Nebraska-bred horse shall mean a horse registered with the Nebraska Thoroughbred or Quarter Horse Registry and meeting the following requirements: (a) It shall have been foaled in Nebraska; (b) its dam shall have been registered, prior to foaling, with the Nebraska Thoroughbred or Quarter Horse Registry; and (c) its dam shall have been continuously in Nebraska for ninety days immediately prior to foaling, except that such ninety-day period may be reduced to thirty days in the case of a mare in foal which is purchased at a nationally recognized thoroughbred or quarter horse blood stock sale, the name and pedigree of the mare being listed in the sale catalog, and which is brought into this state and remains in this state for thirty days immediately prior to foaling.

The requirement that a dam shall be continuously in Nebraska for either ninety days or thirty days, as specified in subdivision (2)(c) of this section, shall not apply to a dam which is taken outside of Nebraska to be placed for sale at a nationally recognized thoroughbred or quarter horse blood stock sale, the name and pedigree of the mare being listed in the sale catalog, or for the treatment of an extreme sickness or injury, if written notice of such proposed sale or treatment is provided to the secretary of the commission within three days of the date such horse is taken out of the state.
